Community Health Collaborating Physician

We are currently hiring collaborating physicians (MD/DO) to support our clinicians performing In-Home Health Evaluations (IHEs). As a collaborating physician, you will earn a monthly stipend based on the number of nurse practitioners and/or physician assistants you oversee.

• Must be licensed and residing in Georgia
• Must also hold a valid license in South Carolina
• Out-of-state clinicians are required to conduct at least one in-home visit or a minimum of five visits annually in South Carolina to meet the collaboration criteria.
• Eligible candidates may receive assistance for airfare, hotel, and rental cars.

Our clinicians provide personalized IHEs to health plan members, which include comprehensive medication and medical history reviews along with physical evaluations in the comfort of their homes. You will be equipped with a pre-populated, patient-specific form for each evaluation, and please note that our physicians do not prescribe medications, order lab tests, or modify existing treatment plans.
