What You’ll Do

Perform in-home client visits : initial sign-ups, 30-day check-ins, and 90-day follow-ups.

Develop and maintain individualized care plans that meet state and federal program requirements.

Review client rights, responsibilities, and advanced directive plans with clients and / or responsible parties.

Document visits in WellSky and the state’s EVV system.

Communicate outcomes with Case Managers and collaborate with Operations Managers on client needs.

Escalate concerns or complex situations to the Director of Operations for support.

Your focus is quality care — not caregiver scheduling. We want you focused on ensuring our clients are safe, supported, and satisfied.

What We’re Looking For

Licensed Practical Nurse (LPN) licensed in South Carolina ( required )

Nursing degree from an accredited program

Current CPR certification ( required )

Minimum age : 21

Experience working with older adults, memory care clients, and adults with disabilities or special needs (acute, residential, long-term care, or home care settings)
