Job Overview :

The Sepsis Coordinator will provide clinical and professional expertise for patients, families and staff members utilizing evidence-based practice to provide education and drive improvement initiatives related to sepsis care. The Sepsis Coordinator will serve as a clinical resource with expertise in sepsis care and will be responsible for collaborating with medical and hospital staff providing care for sepsis patients. This role will require communication and collaboration with mid-level providers, physicians, and the entire multi-disciplinary team to provide patient-centered care, eliminate barriers, and maximize patient outcomes. This person will also work with Quality Analysts to analyze data, and work to improve outcomes related to sepsis care, readmissions, and mortality rates.
