Job Description : -

Provide direct patient care in a long-term care setting under the supervision of an RN or physician.

Administer prescribed medications and treatments accurately and on time.

Monitor and document patient conditions, reporting changes to the supervising nurse / physician.

Assist with daily living activities, ensuring resident dignity and quality of life.

Maintain accurate medical records and ensure compliance with facility policies and state regulations.

Collaborate with the healthcare team to develop and implement care plans.

Ensure infection control and safety protocols are strictly followed.

Required Skills & Experience :

Active New York LPN License

Minimum 2 years of recent LTC experience

BCLS (Basic Cardiac Life Support) Certification
