Position Summary

The Case Manager, Veteran Services is a critical contributor to the Brighton Marine mission, delivering exceptional, quality-driven, comprehensive, veteran-focused services addressing medical, behavioral health, wellness, and social needs, empowering formerly homeless veterans to maintain their permanent supportive housing. The Case Manager is responsible for the delivery of direct support, advocacy, and care coordination for formerly homeless veteran residents of the Brighton Marine Campus and advises on homelessness-related programming in associated locations. The ideal candidate is a Licensed Certified Social Worker (LCSW) who approaches care through all elements of wellness : emotional, occupational, intellectual, environmental, financial, physical, social, and spiritual.

Key Responsibilities

Care Planning and Delivery

  • Conduct biopsychosocial intake assessments to successfully develop and implement individualized care plans, aimed at achieving wellness and maintaining housing.
  • Provide counseling and psychoeducation to veterans based on a tailored care plan, monitoring progress toward improved outcomes and adjusting as needed.
  • Lead the response to urgent mental health needs with targeted crisis intervention support.
  • Plan, coordinate and execute engaging events that promote well-being in all elements of wellness.

Care Coordination & Service Navigation

  • Build relationships with community organizations and service providers offering complimentary resources to veteran residents, keeping abreast of opportunities for positive impact.
  • Coordinate with available resources and service providers to meet veteran resident needs addressing the following areas : medical, substance use, mental health, food insecurity, financial, employment, legal, and social.
  • Serve as an educator, advocate and guide to veterans regarding the available community-based resources, support programs, healthcare, housing, and employment systems with a focus on advocacy and person-centered care.
  • Clinical Quality & Compliance

  • Maintain accurate and compliant case records in adherence to HIPAA and licensing standards.
  • Develop clinical care strategies and program improvements to enhance clinical operations and care efficiency by collecting, analyzing, and reporting on qualitative and quantitative data.
  • Collaborate with internal teams to implement policies and procedures in alignment with quality care expectations.
  • Advocacy & Operations

  • Serve as the internal expert on policies and procedures relevant to veteran services at Brighton Marine with organizations such as : Department of Veterans Affairs, Boston Housing Authority, and other veteran service organizations.
  • In coordination with designated stakeholders, conduct quarterly housing habitability unit inspections and wellness checks to ensure the health and safety of veteran residents.
  • Participate in conferences, attend veteran service and training events, and advise on Brighton Marine’s supportive services programming.
  • Other duties as assigned.
  • Qualifications

    Education & Experience

  • Bachelor’s degree in social work or related field required; master’s degree in social work preferred.
  • Current and valid LCSW license in the state of Massachusetts preferred.
  • Clean driving record and valid Driver’s License required.
  • 2–5 years of case management or clinical experience, preferably with veterans.
  • Familiarity with HUD and VA health system standards preferred.
  • Skills & Competencies

  • Cultural competency in the veteran and military community spaces.
  • Expertise in case management of substance use disorders.
  • Excellent documentation, organizational, and collaboration skills.
  • Strong critical thinking, interpersonal, and problem-solving skills.
  • Maintain a positive, supportive demeanor with veteran residents, and remain calm during difficult circumstances, crisis interventions, and emergencies.
  • Proficiency in case management software and Microsoft Office Suite.
  • Other Requirements

    Availability for some weekend engagement events.

    Physical Nature of the Job

    Some elements of the job are sedentary, but the employee will be required to stand for periods of time, operate a company vehicle, and move throughout the campus, lifting or carrying up to 25 pounds.
