Department : PACE Community Care

All Locations : 155 Addison Street

Position Summary : Includes a $2,000 sign on bonus (pro-rated for part time and per diem)! The PACE Health Aide provides in-home assistance with ADLs and IADLs to Neighborhood PACE participants. Must work independently, problem-solve effectively, and show respect for participants' homes and community. May be assigned to other settings such as the PACE Center.

Essential Duties and Responsibilities :

  • Monitor participant care plans and follow them accurately.
  • Document care provided on flow sheets.
  • Check voicemail at shift start and every two hours; report abnormal findings promptly.
  • Assist participants with ADLs as needed.
  • Accompany participants to appointments.
  • Carry out simple treatments under nursing supervision, like catheter care and skin assessments.

Qualifications and Requirements :

  • High School diploma or equivalent preferred.
  • Current H.H.A. / C.N.A. certification from an approved program.
  • At least one year experience with frail or elderly populations.
  • Experience as a health aide or personal care assistant preferred.
  • Vehicle for travel is a plus.
  • Excellent communication skills; bilingual is a plus.
  • Physical ability to assist with ADLs and mobility.

    Pay Range : $22 to $31 per hour, based on experience
