Hours / Day : 8 (40 hours / week guaranteed)

Weekend Requirements : Rotating weekends; holiday coverage as needed

On Call : No call

Job Setting : Home Health

Responsibilities :

Provide direct physical therapy services to home health clients.

Collaborate with interdisciplinary team members in developing and implementing individualized care plans.

Utilize rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and therapeutic modalities.

Monitor patient progress and adjust interventions as appropriate.

Maintain accurate documentation in compliance with agency and regulatory standards.

Requirements :

Home Health Certified Physical Therapist

2+ years combined acute care (hospital-based) and home care experience; acute care experience preferred

BLS (AHA)
