Job Title : Clinical Site Manager, RN

Location : Sunflower Communities – Nagel Assisted Living

Compensation : $140,000 - $160,000 + 10,000 Sign-on Bonus!

Schedule : Monday - Friday (8 : 00am - 5 : 00pm) + Weekends and On-Call responsibilities.

Job Summary :

Nagel Assisted Living, a part of Sunflower Communities, is seeking a Clinical Site Manager (RN) to oversee the daily clinical operations of our assisted living community. This role ensures that residents receive high-quality care while maintaining compliance with Minnesota Assisted Living License regulations. The Clinical Site Manager will supervise and train staff, collaborate with interdisciplinary teams, and foster a therapeutic, safe, and welcoming environment for residents.

Key Responsibilities :

Clinical Oversight & Compliance :

• Ensure compliance with Minnesota Assisted Living License regulations and be available for state and federal licensure surveys.
• Conduct resident assessments upon move-in and ongoing to ensure their care needs are met.
• Develop, implement, and update care plans and service agreements in a timely manner.
• Oversee medication management , ensuring proper administration, documentation, and compliance.
• Investigate and report incidents of suspected resident neglect or abuse.
• Maintain accurate documentation related to resident care, assessments, and billing.
