Position Overview :
The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. The Advanced Practice Clinician (APC) provides care to high-risk health plan members as part of an interdisciplinary team. Primary Responsibilities :
Conduct comprehensive assessments of membersManage acute and chronic medical and behavioral conditionsEnsure accurate documentation, including ICD-10 codingCollaborate with interdisciplinary care teamsLead advanced illness and care planning conversationsProvide counseling and education to patients and caregiversRequired Qualifications :
Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date)Education :NP : Master of Science in Nursing or Doctor of Nursing PracticePA : Accredited Physician Assistant degree programActive national board certification :NP : AANP or ANCC (Family NP, Adult NP, Gerontology NP, Adult-Gerontology Acute Care NP)PA : NCCPA certificationCurrent DEA licensure / prescriptive authorityAbility to meet physical requirements (lifting 30 lbs, navigating stairs, kneeling for assessments)Reliable transportationAbility to obtain collaborative practice agreement if required by stateFull COVID-19 vaccination (proof required prior to employment)Preferred Qualifications :
1+ year clinical experience (long-term care preferred)Experience managing complex behavioral, social, and functional needsKnowledge of geriatrics, chronic illness, and end-of-life careProficient in electronic medical records and documentationStrong computer skillsAdditional Details :
