Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. You will be responsible for providing high-quality, direct patient care across various settings, focusing on patient comfort, safety, and well-being while adhering to all regulatory standards.

Key Responsibilities

• Clinical Care : Administer medications accurately as prescribed, manage pain protocols, and perform essential medical procedures, including wound care and injections.
• Monitoring & Documentation : Reliably monitor and record patient vital signs . Maintain comprehensive and accurate electronic health records (EHR) , utilizing ICD-10 coding standards for documentation.
• Patient Advocacy : Provide patient and family education on health conditions, treatment plans, and self-care.
• Interdisciplinary Collaboration : Actively collaborate with physicians, Registered Nurses, and other healthcare professionals to develop and execute coordinated care plans.
• Safety & Compliance : Adhere strictly to infection control protocols and maintain a sterile, clean, and organized clinical environment.
• Diverse Care Settings : Provide compassionate care across a range of environments, potentially including outpatient clinics, hospice services, and high-acuity areas like a Level I Trauma Center .
• Lifespan Care : Deliver appropriate care to patients across the lifespan , from toddlers to older adults .
