We're looking for a skilled and caring Registered Nurse (RN) to join our dedicated home care team serving the Rochester, NH community.\n\nIf you are passionate about autonomy, flexibility, and meaningful one-on-one patient care, this is the role for you!\nWhat You'll Do : \n

• Manage a diverse home care caseload with autonomy and flexibility\n
• Perform comprehensive nursing assessments and develop patient-centered plans of care\n
• Collaborate with physicians and interdisciplinary team members to coordinate high-quality care\n
• Monitor patient progress, adjust care plans, and educate patients and families on health management\n
• Complete OASIS assessments with accuracy\n
• Document care accurately and timely during patient visits\n
• Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed\n
• Participate in case conferences and discharge planning\n
• Follow the Nurse Practice Act, agency policies, and home health regulations\n\nWhat You Bring : \n
• Current NH & Maine Registered Nurse (RN) license (or eligibility)\n
• Minimum 1 year of clinical nursing experience preferred (home care experience a plus)\n
• Excellent clinical assessment, communication, and organizational skills\n
• Ability to work independently while being part of a collaborative care team\n
• Comfort working in a variety of home environments\n
