Nurses Network by Quality Home Health Care is a leading provider of home health services in Central and Northern Arizona and has been under new ownership since 2020! We are looking for an ambitious and outgoing individual!

Our Physical Therapists plan is to provide physical therapy services to clients in their homes in accordance with the

Plan of Care and Agency policies and procedures. The Physical Therapist performs functions

that require substantial specialized knowledge, judgment, and skill and must be able to make

judgments accordingly.

Job Details :

  • Our goal will be to fill the role of Home Health PT, and you will join us in our mission of providing top-notch home health care services to our patients.
  • Responsibilities :

  • Follow an individual Plan of Care created by the Case Manager.
  • Provide skilled interventions with a focus on achieving realistic client outcomes within a specified time period.
  • Report significant changes in client status to one of the following Case Manager, Clinical Manager, Director of Nursing, Physician, and other members of the team in a timely manner consistent with client needs.
  • Consistently demonstrate competency with therapy skills according to the personal and legal scope of practice.
  • Practice therapy with respect for individual, cultural, and spiritual differences.
  • Maintain client records showing intervention and education.
  • Ensure accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
  • Demonstrates knowledge of safety / infection control practices by compliance with policies and procedures and regulatory requirements.
  • Promote personal safety and a safe environment for clients and co-workers.

  • Qualifications :

  • Current license as a PT in AZ.
  • Home Health experience.
  • Compliance with professional standards and practice.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Has knowledge of home care federal and state regulations.
  • Licensed driver with automobile insured in accordance with the Agency and state requirements.
  • Has current CPR card.
  • Has TB test (performed within 6 months of hire) or signed release from MD w / CXR stating you're free from s / s of TB
