NP / PA - Rural Health Initiative - Shawano, New London, Appleton, WI

ThedaCare  is seeking an experienced  Nurse Practitioner or Physician Assistant  to provide comprehensive, coordinated care in  Appleton, New London, and Shawano, Wisconsin.

Details :

1.0 FTE

Best of both worlds seeing patients in their homes & via telehealth. Company car and all technology needed is provided.

36 contact hours

4 on days in-person in patient homes.  12 on days telehealth

Registered Nurses, Community Health Workers (CHW)-will be in the patient home to facilitate the APC telehealth visits, Community Paramedic to run any rapid need interventions.

BLS / ACLS / PALS Required

ATLS Required
