Description of the role:

Interim HealthCare in New Hampshire is seeking a compassionate Licensed Practical Nurse (LPN) for home care services in Manchester, NH. The ideal candidate will provide skilled nursing care to patients in their homes, focusing on helping them achieve the highest possible level of independence and quality of life.

Responsibilities:

• Conduct initial assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Monitor and report changes in patient's condition
• Educate patients and families on self-care techniques
• Collaborate with other healthcare professionals to provide comprehensive care

Requirements:

• Active Licensed Practical Nurse license in the state of New Hampshire
• Minimum of 1 year of nursing experience
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
