You'll : \n

• Create Individualized Plans based on physical, emotional, and environmental assessments.\n
• Provide Hands-On Care that prioritizes comfort, symptom management, and quality of life.\n
• Collaborate with Confidence by communicating vital updates to physicians, supervisors, and your interdisciplinary team.\n
• Support Families Compassionately, guiding them through the hospice journey with education and empathy.\n
• Act Proactively, quickly responding to any changes in patient condition and adjusting care plans as needed.\n
• Deliver Excellence with every visit, upholding the highest standards of clinical care and patient dignity.\n\nWhat You Bring\n
• Graduation from an accredited nursing program (Diploma, ASN, or BSN)\n
• Current RN license in Michigan\n
• Minimum 1 year of clinical nursing experience\n
• 1 year of Hospice or Home Health experience is required\n
• Reliable transportation and a commitment to patient-centered fieldwork\n
• Ability to multitask, document thoroughly, and adapt with grace\n
