You'll : \n
- Create Individualized Plans based on physical, emotional, and environmental assessments.\n
- Provide Hands-On Care that prioritizes comfort, symptom management, and quality of life.\n
- Collaborate with Confidence by communicating vital updates to physicians, supervisors, and your interdisciplinary team.\n
- Support Families Compassionately, guiding them through the hospice journey with education and empathy.\n
- Act Proactively, quickly responding to any changes in patient condition and adjusting care plans as needed.\n
- Deliver Excellence with every visit, upholding the highest standards of clinical care and patient dignity.\n\nWhat You Bring\n
- Graduation from an accredited nursing program (Diploma, ASN, or BSN)\n
- Current RN license in Michigan\n
- Minimum 1 year of clinical nursing experience\n
- 1 year of Hospice or Home Health experience is required\n
- Reliable transportation and a commitment to patient-centered fieldwork\n
- Ability to multitask, document thoroughly, and adapt with grace\n
