Description :

This position entails managing a collaborative process to assess, plan, implement, monitor, and evaluate options and services required to meet patients' health needs.

The Care Manager will utilize effective communication and available resources to promote quality and cost-effective outcomes for a patient load of 18-22.

Responsibilities include meeting patient needs, managing length of stay, and promoting the efficient utilization of resources.

The Care Manager will determine medical necessity and the appropriateness of admission using established screening criteria.

Extensive knowledge of current patterns of practice and acute inpatient care is required.

The role requires familiarity with methods for assessing physical and mental impairment, as well as an understanding of illness characteristics and expected treatment.

Knowledge of benefits, plan contracts, eligibility issues, and requirements is essential.
