Palliative and Skilled Care RN - Hybrid

SALMON Home Care seeking a qualified Registered Nurse to provide comprehensive nursing care to patients receiving palliative and skilled home health services through our Visiting Nurse Association. This hybrid role combines direct in-home patient visits with remote phone-based follow-up, education, and care coordination. The RN works closely with the interdisciplinary team to promote optimal patient outcomes, manage symptoms, and support patients and families in achieving their care goals.

32 hours / week- Hybrid
