As a Nurse Practitioner, you will conduct thorough health assessments, manage chronic and acute conditions, develop individualized care plans in collaboration with an interdisciplinary team, and provide education to residents and their families. You will also participate in quality improvement initiatives to enhance resident outcomes and maintain accurate documentation in the electronic medical record system.

Requirements
- Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program
- Unrestricted New York State Nurse Practitioner license
- Valid DEA permit allowing prescription of medications commonly used in practice
- Eligible for Medicare and Medicaid provider status and reimbursement
- Minimum of 3 years clinical experience, preferably in geriatrics or long-term care
- Experience with eClinicalWorks (eCW) electronic health records preferred
