Registered Nurse (RN) - Acute Care Specialist

You will play a crucial role in promoting patient- and family-centered care, utilizing evidence-based practices throughout the nursing process : assessment, planning, implementation, evaluation, and outcomes.\n\nThis position demands agility, as staff may be scheduled for shifts that include weekends, holidays, and on-call duties.\n\nKey Responsibilities\n

• Conduct thorough nursing assessments, collecting data and identifying patient needs using appropriate standards and tools.\n
• Develop individualized care plans in collaboration with patients, families, and the interdisciplinary healthcare team, reviewing and adapting as patient conditions change.\n
• Set realistic goals and outcomes for patient discharge, ensuring continuity of care beyond hospitalization.\n
• Respect and incorporate the cultural, spiritual, and personal beliefs of patients and families in care planning.\n
• Provide direct nursing care based on established plans while effectively delegating tasks to team members.\n
• Monitor and enhance the quality of care, ensuring a safe and supportive environment for every patient.\n
• Evaluate patient responses and adjust care plans accordingly, collaborating with healthcare team members for optimal outcomes.\n
• Identify learning needs for patients and families, facilitating effective teaching tailored to their understanding.\n
• Act as a patient advocate, coordinating care seamlessly across the healthcare continuum.\n
• Engage in ongoing professional development to maintain competency and licensure.\n
• Perform additional duties as assigned to enhance patient care and departmental efficiency.\n\nQualifications\n
• Education : Associate degree in Nursing; Bachelor's degree preferred.\n
• Experience : A minimum of one (1) year of nursing experience. New graduates may be eligible through successful completion of a Nurse Residency Program.\n
• Certification : Current RN licensure recognized by the NCSBN Compact State or South Carolina.\n\nThis is a non-management position reporting to a supervisory or managerial role.
