You will play a crucial role in promoting patient- and family-centered care, utilizing evidence-based practices throughout the nursing process : assessment, planning, implementation, evaluation, and outcomes.\n\nThis position demands agility, as staff may be scheduled for shifts that include weekends, holidays, and on-call duties.\n\nKey Responsibilities\n

- Conduct thorough nursing assessments, collecting data and identifying patient needs using appropriate standards and tools.\n
- Develop individualized care plans in collaboration with patients, families, and the interdisciplinary healthcare team, reviewing and adapting as patient conditions change.\n
- Set realistic goals and outcomes for patient discharge, ensuring continuity of care beyond hospitalization.\n
- Respect and incorporate the cultural, spiritual, and personal beliefs of patients and families in care planning.\n
- Provide direct nursing care based on established plans while effectively delegating tasks to team members.\n
- Monitor and enhance the quality of care, ensuring a safe and supportive environment for every patient.\n
- Evaluate patient responses and adjust care plans accordingly, collaborating with healthcare team members for optimal outcomes.\n
- Identify learning needs for patients and families, facilitating effective teaching tailored to their understanding.\n

- Education : Associate degree in Nursing; Bachelor's degree preferred.\n
- Experience : A minimum of one (1) year of nursing experience. New graduates may be eligible through successful completion of a Nurse Residency Program.\n
