Willow Valley Communities LPN wages are determined on an experience based pay scale.
Wages :
$27.00-$33.00 / hr, based on experienceWeekend LPN $35.00 / hrResponsibilities :
Administers treatments and medication as specified in MAR, TAR, PRN sheets and follow properlyAssembles and uses equipment including catheters, tracheotomy tubes and oxygen suppliersAssists with providing guidance and assistance to Residents as needed with ADLsReviews 24 hour report and general notes in PCC from the preceding shiftFollows through on unfinished orders / tasks from shift priorAssures Resident care is completed by assignmentsDocuments changes in Resident care and conditionMaintains calendar of Resident appointments and assures that transportation of Residents to appointmentAssesses Residents' condition and informs physician, family / responsible party of any changes and plan of action / treatment, as neededReceives orders from physicians and brackets order with time, date and initials. Transfers order to PCCNotifies pharmacy of changes in medication orders or need for refills, initial and date order changesComplies with Department of Human Services regulations and follows the WVC policies and proceduresCompletes lab requests per physician orders with all pertinent information.Assures that Residents are seen by a physician annually for medical evaluation or when change iResident's condition and that physician completes a DME.Assures a RASP is completed with each DME.Assures that physicians complete necessary paperworkQualifications :
Valid PA Practical Nurse (LPN) License required.Hours per week :
Full Time and Part Time availableSchedules available :
