SUMMARY :
Family Service League is seeking a full-time Case Manager for the Childrens Health Homes program. The Case Manager will offer care coordination to children and young adults with mental illness and / or chronic medical conditions. The Case Manager will be responsible for conducting meetings with clients to develop strength assessments and individualized planning, as well as support them with their goal achievements. Travel is required.
SCHEDULE :
Monday-Friday 9 : 00 AM-5 : 00 PM, flexible schedule
Evening hours required for at least 2 days a week

QUALIFICATIONS
Bachelors Degree in Social Work with 2 years of related experience or Masters Degree with 1 year of related experience required.
Excellent interpersonal and organizational skills required.
Strong verbal and written communication skills required.
Ability to solve problems and work as part of a team required.
Proficient computer skills, including Microsoft Office, required.
Valid and clean NYS Drivers License required.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time and travelling to conduct home visits.
