2 Skilled communities with attached Assisted Living 6-8 hours a week at each with remote support between weekly visits.

If willing to travel 1 - 1 1 / 2 hour radius twice a week - we could provide full time work status

Must have NDTR / RD credential awarded from the Commission on Dietetic Registration.

KEY RESPONSIBLITITES MAY INCLUDE, BUT NOT LIMITED TO :

• Completion of MDS assessments, Section K, CAA
• Documentation for residents at a high risk for nutritional problems.
• Participation in the care planning process.
• Collaboration with the Interdisciplinary Team.
• Provide oversight to the food service department with routine audit for safety, sanitation, and quality of food.
