Care Guide Representative

A company is looking for a Care Guide Representative to facilitate patient care and support health goals.

Key Responsibilities

Acts as the central facilitator to patients, coordinating care and communication

Identifies and resolves barriers to patient health care goals

Develops patient-driven goals and supports adherence to care plans

Required Qualifications

Must be 18 years of age with relevant education and / or experience

0 to 2 years of experience in customer service, health care, or community-based roles

High school diploma or GED preferred

Bachelor's degree in health care is a plus
