Healthcare United is seeking a Board-Certified Internal Medicine or Family Medicine Physician to join a rapidly growing and well-regarded healthcare system located just 20 minutes from Franklin County, Ohio. This is an exceptional opportunity for a dedicated primary care provider who is passionate about team-based care and making a meaningful impact on population health outcomes.

Position Overview :

Setting : 100% Outpatient

Schedule : Monday Friday, 8 : 00 AM 5 : 00 PM

Call : Light call schedule (1 : 3), no weekends

Patient Population : Adults with chronic and complex conditions

Team-Based Care : Collaborate closely with social work, case management, behavioral health, pharmacy, and nutrition

Care Coordination : Monitor hospital / ER discharges, SNF transitions, and home care

Focus : Prevent avoidable ER visits, hospitalizations, and readmissions through proactive disease management

Requirements

Board Certified in Internal Medicine or Family Medicine

Active Ohio medical license (preferred but not required for application)

BLS Certification

Experience in population health management strongly preferred

Strong communication and coordination skills in a multidisciplinary care environment
