Job Location : Muskogee, OK 74401

Job Duration : Direct Hire

Shift / Schedule : Day Shift | Monday - Friday, 8 : 00am - 4 : 30pm

Job Responsibilities :

• Assess patients and families referred to Social Work services in a timely manner.
• Integrate social work plans into overall patient care through interdisciplinary collaboration.
• Provide skilled services including crisis intervention, case management, discharge planning, advocacy, and patient / family education.
• Manage psychosocial issues such as abuse / neglect, advanced directives, psychiatric placement, and end-of-life concerns.
• Utilize hospital and community resources to ensure continuity and quality of care.
• Refer patients, families, and staff to appropriate services; assist with insurance coverage for services.
• Document care plans, interventions, and outcomes accurately in the medical record.
• Complete statistical reports as required.
• Collaborate with interdisciplinary teams on complex discharge and transitional care plans.
• Participate in professional development, community education, and support group activities.
• Exercise independent judgment when making decisions with minimal review.
• Work directly with patients, families, hospital staff, internal and external customers, and healthcare leadership.
