About The Position

If yes, please provide explanation.Has the candidate ever been convicted of a crime other than a minor traffic violation?Does the candidate have an active BLS issued through American Heart Association?Is the candidate licensed to practice as an Occupational Therapist in the State of California? Must include candidate's education (Including month / year of graduation), & complete work history from the last seven (7) years. All work history should include facility worked at, not the agency name.Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submittedTwo (2) References Two (2) references from work history occurring within the past three (3) years. At least (1) one reference must be from a supervisor.Online verification of professional license or certification Verified within 30 days of submission. Nursys acceptable.Online Verification of Professional License or Certification to practice in the State of California Online verification of professional license completed within thirty (30) days of start and at time of extension. For RN / LPN : verification must also include a Nursys report, uploaded together.Drug Screen 9-panel drug screen collected within one (1) year of start date; required annually. Rapid or instant drug screens are not accepted. Drug screen must include the following panel : Amphetamine to include Methamphetamine, Barbituates, Benzodiazepines, Cocaine, Methadone, Methaqualone, Propoxyphene, Opiates / Opioids, Phencyclidine. Drug Screens that list marijuana are not accepted. An Affiliate attestation form can be accepted in lieu of drug screen results.BLS for Healthcare Providers Must be issued through American Heart AssociationPhysical Completed within one (1) year of start date; required annually. May be signed by Physician, NP, PA, DO, or DC.MMR Shown by proof of two (2) MMR vaccinations or positive IGG titers. Decline form accepted for low / equivocal titers, Affiliate form acceptable.Varicella Shown by proof of two (2) vaccinations, positive IGG titer, or provider verified history. Decline form accepted for low / equivocal titers, Affiliate form acceptable.Hepatitis B Shown by proof of vaccination series, positive surface antibody titer, or declinationInfluenza For Assignments occurring between 11 / 1-3 / 31. Influenza Vaccine or Flu Shot Declination. If the CP is declining the Influenza Vaccine, they must complete the NMC Influenza Declination Form (attached) and follow masking protocol in patient care areas.Tdap Administered within ten (10) years of start date. Decline form accepted, Affilate form acceptable.TB TB skin test, TB QuantiFeron, or T-Spot completed within one (1) year of start date and annually thereafter. Chest X-Ray within one (1) year of start date for past positive TB case as a result of TB exposure. Past positive documentation, Chest X-Ray, and TB questionnaire required. TB questionnaire required annually.Background Check Completed within thirty (30) days of start date and INCLUDE ALL ALIASES. 1) National Criminal Check; 2) FACIS Level III; 3) National Sex Offender; 4) ALL counties lived and worked within past 7 years; 5) SSN Address TraceOffice of Inspector General (OIG) OIG completed within thirty (30) days of start date, and at time of extension. Photo ID must be submitted to clients for identification verification.California (CA) Meal Waiver Form Client form is requiredDepartment Notes : 28 bed unit, inpatient Rehab unitMisc like OT with a minimum of 1 year experience as a Occupational Therapist.  Will consider new graduates as well.  Will need to be able to step in and provide treatments in an inpatient setting.  Requirements :   Licensed as an Occupational Therapist in the State of California and BLS issued through American Heart Association required. Some weekend coverage required. Charting : MeditechDress code :   May wear any color of professional scrubs if working on the Inpatient side.  No other facility names or logos are permitted to be on scrubs.  Orientation :   Ability to start any day of the week.  Therapist will complete an orientation to the hospital, department and floor they will be working on. They are paired with a buddy to help orient and train for the first couple of days then when ready and cleared by manager they begin treating patients on their own. Guaranteed Hours : 40 Contract Weeks : 88
