Personal injury Case Manager

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will possess a strong background in behavioral health and social work, with a focus on providing support and resources to individuals in need. As a Case Manager, you will play a crucial role in crisis intervention, workforce development, and child welfare, ensuring that clients receive the appropriate services to improve their quality of life.

• Responsibilities
• Conduct comprehensive assessments of clients to identify needs and develop individualized service plans.
• Provide crisis intervention and management to clients facing immediate challenges.
• Collaborate with healthcare providers, social services, and community organizations to coordinate care.
• Utilize motivational interviewing techniques to empower clients in their recovery journey.
• Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
• Offer mentoring and support to clients navigating addiction counseling or behavioral health issues.
• Maintain accurate documentation of client interactions, progress notes, and service outcomes.
• Stay informed about Medicare regulations and other relevant policies impacting client services.
• Experience
• A degree in Social Work, Psychology, or a related field is preferred.
• Previous experience in crisis management or behavioral health settings is highly desirable.
• Familiarity with workforce development initiatives and child welfare practices is a plus.
• Strong interpersonal skills with the ability to build rapport with diverse populations.
• Knowledge of motivational interviewing techniques and best practices in case management.
• Experience in mentoring or providing support to individuals facing addiction challenges is beneficial.
