Schedule : Full-time, Mon–Fri, ~8 a.m.–5 p.m.

About the Role

Join our Care Anywhere (CAW) team delivering proactive, in-home medical and social support to seniors. You'll provide 4–5 home visits per day, complete geriatric assessments, support ongoing care, and help prevent avoidable hospitalizations. This role blends autonomy in the field with collaboration across an interdisciplinary team.

Responsibilities

• Conduct in-home assessments, screenings, and medication reviews
• Provide follow-up visits and continuity of care
• Educate patients / caregivers on disease management and prevention
• Coordinate with PCPs, specialists, and internal teams
• Complete timely EMR documentation
• Travel within ~25-mile radius; mileage reimbursed
