Essential Responsibilities
Provide and oversee daily clinical care of residents in assigned skilled nursing facilities.Conduct comprehensive histories and physicals, diagnose, and develop treatment plans (including diagnostics, prescriptions, and monitoring).Offer patient counseling and education on health maintenance and disease prevention.Deliver high-quality, patient-centered primary care in the SNF environment.Complete accurate, timely documentation of all encounters.Be available for phone or secure messaging consultations with nursing staff as needed.Use facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech (training provided).Collaborate with supervising physicians when applicable, following established clinical protocols.Participate in chart reviews and quality audits to support ongoing clinical improvement.Maintain reliable transportation and punctual attendance for all scheduled shifts.Qualifications
Must be Medicaid-enrolled in the state of employment.Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in good standing.Strong clinical skills in primary care, post-acute, or skilled nursing settings.Excellent communication and collaboration skills.Commitment to patient-centered care and continuous improvement.Compensation & Benefits
