Job descriptionConsistent weekday schedule : Monday through Friday, daytime hours Generous PTO and paid holidays Immediate health insurance upon hire Free dental insurance with optional upgrades 401(k) with employer match A supportive and professional team environment Work as part of a multidisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants Deliver care in multiple settings : Participant homes Partner skilled nursing facilities Participate in a rotating after-hours on-call schedule to support the on-call nurse Build strong relationships with participants and their families through education and support Practice in a flexible care model that adapts to participant needs rather than rigid systems A passion for serving others with a positive, person-centered approach Commitment to interdisciplinary decision-making that prioritizes : Respect for self-determination Graduate of an accredited Nurse Practitioner or Physician Assistant program Current and valid Michigan license as a Nurse Practitioner or Physician Assistant Strong clinical leadership skills Excellent interpersonal and communication skills Effective problem-solving abilities A collaborative mindset with a focus on shared decision-making Experience in primary care or geriatric care (preferred) An inspirational, motivational, and creative approach aligned with the PACE model of care
