PACE North is seeking a compassionate and dedicated Nurse Practitioner or Physician Assistant who is motivated to make a meaningful impact in the lives of older adults in our community.

- A unique care model for individuals aged 55+ who qualify for nursing facility-level care but prefer to live at home

- Focused on providing personalized care in a variety of settings : in our clinic, in the home, or in skilled nursing facilities

Work as part of a multidisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participantsDeliver care in multiple settings :On-site clinic
Participant homesPartner skilled nursing facilitiesParticipate in a rotating after-hours on-call schedule to support the on-call nurseBuild strong relationships with participants and their families through education and supportPractice in a flexible care model that adapts to participant needs rather than rigid systemsRequirements :
