Travel Nurse RN - Case Manager - $1,990 per week in Baltimore, MD

Job Title : Case Manager Profession : RN Specialty : Case Management Duration : 13 weeks Shift : Day Hours per Shift : 8 : 30 AM - 5 : 00 PM Experience : 2 years RN experience License : Active RN License Certifications : BLS Must-Have : Current in-patient Case Management experience; EPIC and post-acute facility referral platforms experience Description : The Case Manager coordinates care for a select caseload of patients from triage to care coordination and safe transfer home. Proactively communicates with payors to ensure approval of treatment plans and required resources. Maintains knowledge of and responds to clinical systems and fiscal data related to specific patient populations. Collaborates with the multi-disciplinary team and patients and their families to ensure effective transition planning. Coordinates transitions to post-acute settings, including nursing facilities and acute inpatient rehab. Manages patients based on departmental procedures while maintaining clinical safety and quality. Enters and maintains patient information in all departmental systems, per operational guidelines. Bedside Case Managers are required to have a current / active CPR card. They do not need to complete the Med Test since they will not have access to or pass medications to patients.
