WeCare at Foltsbrook is actively seeking motivated Licensed Practical Nurses for our community in Herkimer, NY. Our LPNs assist in ensuring the general health and well-being of our residents by providing nursing care. The  LPN provides nursing care under the supervision of a registered nurse (RN) or physician to residents. The LPN assists in implementing the nursing care plan, administering medications, and providing basic medical care to residents.

Key Responsibilities

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.
