- Some travel is involved. Meetings can occur in Richmond, Virginia Beach, Suffolk, Norfolk, Newport News, Hampton, Chesapeake, etc.
 
General Responsibilities : The individual will be responsible for providing QDDP Services for adults and children with intellectual and developmental disabilities.
The QDDP assumes responsibility for the following roles to include :
Records managementRecords review / auditsConducting and approving assessmentsPrescreening, admitting, and discharge planningIntake screening and associated paperworkAttend Intake, SIS, and Annual meetingsOverseeing the development and implementation of Individualized Service Plans (Part V Initial and Annual)Writing reports such as Quarterly Service ReviewsReviewing documentation on the individualsCoordinating services with Support coordinator to include scheduling meetingsKnowledge, Skills, and Abilities :
Experience managing services for adults and children with intellectual, developmental, physical, and mental disabilitiesExperienced in personnel management, HIPPA and records management, developing and implementing Individualized Service Plans, conducting, and approving assessments, prescreening, and discharge planningFamiliarity with regulations governed by DBHDS and Offices of Human Rights, Medicaid, Department of Labor, OSHA, Community Service Boards, Case Management Services, First Aid, and CPREffective written and oral communication skillsStrong documentation skillsKnowledge of behavioral plansQualifications :
A Bachelor’s degree plus one year of experience and supervisory skills working with the intellectually and developmentally disabled population servedMust have a valid Virginia driver’s licenseMust have one (1) year of working with the intellectually and developmentally disabled populationCertifications are preferred for Therapeutic Options, First Aid, and CPR. Will provide this training if necessary.
