Description : Occupational Summary
The Hospice Nurse Practitioner is a licensed advanced practice registered nurse (APRN) who provides comprehensive hospice services in accordance with the state Nurse Practice Act. Under the supervision of a collaborating physician, the Nurse Practitioner conducts patient visits to perform face-to-face assessments and evaluate ongoing hospice eligibility in collaboration with the interdisciplinary team.
Primary Responsibilities
- Conduct face-to-face assessments prior to the third and subsequent benefit periods to evaluate continued hospice eligibility.
- Provide clinical findings and recommendations to the Hospice Medical Director for certification and recertification decisions.
- Maintain up-to-date knowledge and compliance with Medicare, Medicaid, and state / federal hospice regulations.
- Ensure adherence to HIPAA and all confidentiality standards.
- Participate in quality assurance and performance improvement activities.
- Engage in professional development through continuing education, seminars, and in-service training.
- Accept and complete assigned duties in a timely and responsible manner.
- Other duties as assigned.
Requirements :
Graduate of an accredited School of Nursing and Nurse Practitioner program.Master’s degree in Nursing.Minimum of one (1) year Nurse Practitioner experience.Minimum of one (1) year experience in hospice or palliative care.Certification from an approved advanced practice nursing board as required by state law.Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.Current CPR certification.Proficient in English (reading, writing, and speaking).Strong observational, communication, and problem-solving skills.Ability to meet deadlines and work independently.Capable of performing CPR and managing airway obstructions.Physically able to climb at least two flights of stairs and stand or walk for up to four hours.Valid driver’s license and proof of current vehicle insurance.Satisfactory motor vehicle driving record as required by the agency and insurance providers.
