Job description

Medical Director (Family Medicine or Emergency Medicine or Internal Medicine) Post-Acute Care

Seeking an experienced BE / BC Family or Internal or Emergency Medicine physician to provide primary care in skilled nursing facilities.

Position Details :

Full-time - 3.5 days - 5 days / week
Flexible Monday Friday schedule
Employed, Permanent position
See up to 20 patients per day

Responsibilities :

Primary care rounds, chronic disease management, care planning

Collaborate with facility staff and patients' families

Provide leadership, guidance, and clinical oversight
