Job Description
Acclaim Multi-Specialty Group is seeking a Full Time Advanced Practice Provider (APP) to join the Correctional Medicine Team at the Tarrant County Jail in downtown Fort Worth, TX. The APP is responsible for all aspects of patient care, including acute and chronic care diagnosis, treatments and consultations. The provider will also provide phone consultations with two other correctional facilities.
Schedule – 12 hour shifts (7 : 00 pm - 7 : 00 am) on a rotating basis including weekends.
Under the supervision of the collaborating physician, the APP reviews patient medical charts and conducts physical exams to diagnose, treat and manage care of patients. Makes appropriate patient referrals as necessary. Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by the Tarrant County Department of Corrections, JPS Health Network and Acclaim Physician Group. Provides care education to patients. Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes. Maintains a collaborative environment with other providers, clinical staff, and support staff. Performs other job related duties as assigned.

Required Education and Experience
Graduate of an accredited Physician Assistant Program or Nurse Practitioner Program. Minimum of 2 years of experience working as an APP in a primary care setting.
Required Licensure / Certification / Specialized Training
Must have current Texas Physician Assistant license or Texas APRN license (Family Nurse Practitioner (FNP-BC) or Adult Nurse Practitioner certification (AGPCNP-BC)). Basic Life Support (BLS) certification. Advanced Cardiac Life Support (ACLS) certification.
Preferred Experience
Prior experience working in a large county jail system as a Physician Assistant or Nurse Practitioner, including clinical rotations.
