RN Case Manager (Atlantic County)
Salary : $32.26 - $43.68 based on years of experience
A compassionate and dedicated job-seeker is needed for the Hospice Nurse opportunity with our company! As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients.

They provide an individualized orientation and training program!
What You'll Do :
- Perform patient assessments for comfort and relief of symptoms.
- Establish and implement an individualized plan of care for each patient.
- Assess the need for patient and family education and provide education as needed.
- Obtain appropriate physician orders to achieve symptom management.
- Triage and arrange a home visit schedule, demonstrating efficiency in time and mile management.
- Make referrals to additional IDT members and collaborate with social workers related to their involvement.
