Description : CAP Case Manager – Hybrid Remote
Job Type : Full-time, Hourly
Pay Range : $18.75 – $20.00 / hour

Position Summary
Healthkeeperz is seeking a compassionate and detail-oriented CAP Case Manager to support the Community Alternatives Program (CAP) for Disabled Adults and Children.
In this role, you will :
Conduct assessments and determine eligibility for CAP servicesDevelop and implement individualized care plansCoordinate services with families, providers, and community resourcesPerform regular home visits and follow-upsMaintain accurate documentation in compliance with CAP and state guidelinesThis position combines fieldwork, office time, and remote flexibility. Core Responsibilities
Assessment & Planning : Complete patient screenings, identify needs, and create individualized care plans.Coordination : Partner with families, providers, and agencies to ensure appropriate services.Follow-Up : Conduct home visits (35–40 per quarter), monitor progress, and adjust plans as needed.Documentation : Maintain accurate records in the E-CAP system, review aide notes, and support compliant billing.Professional Growth : Complete continuing education and stay current with CAP program requirements.Hybrid Work & Travel
Hybrid Schedule : Based at the Durham office, with the option to work remotely up to two days per week after onboarding and supervisor approval.Field Travel : Regular home visits across Durham County (private homes and occasional facilities).Schedule Flexibility : Case Managers set their own visit schedules while meeting program requirements.Vehicle Use : Must provide your own transportation; mileage reimbursement is provided.Requirements : Requirements
Education / Experience (one of the following) :
Bachelor’s in Social Work + 1 year related experience, orBachelor’s in Human Services + 2 years related experience, orBachelor’s in Non-Human Services + 2 years related experience, orCurrent NC RN license + 1 year related case management experience, orApprentice / Trainee (Bachelor’s or Nursing license without full experience) – requires direct supervisionAdditional Skills :
Knowledge of casework and social service practicesFamiliarity with community resources and agenciesStrong communication and organizational skillsAbility to travel for visits and meetingsComfort with electronic documentation and virtual communication toolsWork Environment
This position involves travel in rural areas, varied home environments, and occasional exposure to unpredictable conditions. Employees must be comfortable with independent fieldwork and adaptable to changing needs.
