Description :

This position involves case management in a healthcare setting.

The unit primarily consists of 20 beds.

The team includes case managers and social workers to manage the patient load.

Day shift work is available from Friday to Monday, with hours from 7 : 30 AM to 3 : 30 PM, and Monday to Friday from 8 : 00 AM to 4 : 30 PM.

The patient-to-social worker ratio is 1 : 40.

Daily responsibilities include performing suicide risk assessments and substance use assessments.

Discharge planning assessments are a key component of this role.

Candidates who have experience in emergency department settings are preferred but not required.

Knowledge of palliative care and goals of care conversations is a plus.

Ideal candidates should be professional, dedicated, friendly, and possess strong communication skills.

Adaptability to different units is essential for this role.
