Position Summary :
The Home Health Aide will report directly to the Director of Nursing. The home health aide monitors or reports changes in health status and provides personal care such as bathing, dressing, and grooming of the patient.
Key Responsibilities :
- Assist patients with personal care activities such as bathing, grooming, dressing, and toileting.
- Monitor patients’ condition by observing physical and mental condition, intake and output, and exercise.
- Assist with meal preparation, feeding, and medication reminders.
- Perform light housekeeping duties such as laundry, washing dishes, and changing bed linens.
- Provide companionship and basic emotional or psychological support.
- Maintain accurate records of services performed and report changes in patients' conditions to a nurse or supervisor.
- Follow the care plan outlined by the care team and participate in updating care plans when necessary.
- Perform other duties as assigned.
Physical and Environmental Requirements :
Work is performed in clients' homes and may involve travel between client homes.May involve exposure to infectious diseases and bodily fluids.Requires bending, lifting, standing for extended periods, and the ability to assist with mobility.Additional Expectations :

High school diploma or equivalent.Qualifications :
Certification from a state-approved Home Health Aide training program.Current CPR / First Aid certification (preferred).Previous experience in home health or personal care preferred.Strong communication and interpersonal skills.Physical stamina and the ability to assist clients with mobility.Compassionate and patient demeanor.US work authorization (Required)Salary : $16 / hr
