About This Role

This is a special opportunity for a mission-driven Licensed Vocational Nurse (LVN) who’s passionate about helping older adults live safely, comfortably, and independently at home. As an Enrollment LVN, you’ll lead the way in conducting initial nursing and home safety assessments for prospective participants exploring the PACE program — helping each individual take the first step toward receiving the coordinated, compassionate care they deserve.

As a valued member of our Enrollment and Outreach Team, you’ll serve as a vital bridge between the Interdisciplinary Team (IDT) and the Outreach Team, ensuring a smooth, informed, and supportive enrollment experience from start to finish. Through your clinical expertise, caring presence, and attention to detail, you’ll identify the right level of care for each participant and ensure every home environment is safe, welcoming, and ready for their PACE journey ahead.

What Does Success Look Like As Our Enrollment LVN Role?

  • Bring care to their doorstep : Conduct home visits to evaluate prospective participants for nursing home level of care and assess the safety, accessibility, and overall condition of their home environment.
  • Make homes safer and stronger : Perform comprehensive home safety evaluations to identify potential hazards, fall risks, and accessibility challenges — offering thoughtful recommendations for modifications and assistive devices.
  • Collaborate for better outcomes : Partner closely with the Outreach Specialist and Interdisciplinary Team (IDT) to support timely, accurate enrollment decisions and contribute to Enrollment Eligibility meetings.
  • Assess care needs early : Conduct preliminary home care assessments to determine caregiver support levels and potential service hours for each participant prior to enrollment.
  • Empower through education : Provide guidance to participants, families, and caregivers on home safety, fall prevention, and strategies for aging gracefully in place.
  • Keep every detail documented : Maintain accurate, timely records of all evaluations, assessments, and recommendations within CRM and electronic health record (EHR) systems.
  • Champion compliance and quality : Participate in PACE audits conducted by DHCS and CMS, ensuring all standards and program requirements are met with excellence.
  • Support growth and satisfaction : Partner with the Executive Director and Enrollment Team to enhance participant retention and overall satisfaction.
  • Be an ambassador in the community : Assist with outreach and business development activities to promote PACE and help more seniors access our life-changing care model.
  • Stay sharp and informed : Keep current on PACE eligibility criteria, enrollment procedures, and evolving regulatory standards.
  • Contribute wherever needed : Take on additional tasks to ensure the Enrollment Department — and our mission — continue to thrive.

What Does An Ideal Profile Look Like?

  • Licensed and ready to serve : Active, unrestricted Licensed Vocational Nurse (LVN) license in the state of California.
  • Experienced in care coordination : Minimum 2 years of experience providing direct care or conducting clinical assessments with medically complex or older adult populations, preferably in home health, community health, or PACE settings.
  • Confident on the road : Valid California driver’s license, reliable personal transportation, proof of auto insurance, and a willingness to travel to participants’ homes for evaluations.
  • Strong communicator and collaborator : Excellent interpersonal skills with the ability to build trust and communicate effectively with participants, families, and interdisciplinary team members.
  • Tech-savvy and detail-oriented : Proficient in using   EHR and CRM systems, with strong documentation and organizational skills to manage multiple cases at once.
  • Compassion meets compliance : Understanding of PACE program eligibility, DHCS / CMS regulations, and a commitment to maintaining the highest standards of care and documentation.
  • Safety-minded and proactive : Demonstrated ability to identify risks in home environments and recommend appropriate interventions to promote participant safety and independence.
  • Adaptable and mission-driven : Thrives in a fast-paced, community-based healthcare environment, demonstrating flexibility, empathy, and dedication to helping seniors age in place with dignity.
  • What Does The Working Environment Look Like?

  • Out in the community : This is a  field-based role with extensive travel throughout the assigned PACE service area, you’ll be the friendly face of myPlace Health, meeting participants where they are.
  • Every visit makes an impact : Conduct home visits and community-based assessments to evaluate safety, accessibility, and level of care needs — helping participants live independently and confidently.
  • A mix of settings : Work takes place primarily in participants’ homes, community locations, and occasionally at the PACE center for meetings or documentation.
  • On the move : Expect frequent driving across various neighborhoods, sometimes through heavy traffic, as you bring care and connection directly to our participants.
  • Active and hands-on : Home visits may involve walking, standing, bending, and climbing stairs as you perform safety evaluations and environmental assessments.
  • Adaptable to every environment : Be comfortable entering a variety of home settings, some with pets, unique odors, uneven surfaces, or limited accessibility.
  • Balance activity and focus : You’ll spend time both on your feet and seated for extended periods while driving between visits or completing documentation.
  • Prepared and professional : Occasionally lift or carry clinical supplies or equipment (up to 25 pounds) as part of your fieldwork.
  • Weather won’t stop you : Expect exposure to outdoor conditions such as heat, rain, or cold when traveling between homes or community sites — safety always comes first.

  • Schedule you can count on : Monday through Friday, with occasional weekend coverage based on participant or program needs.
  • Hybrid flexibility : While much of your time will be spent in the field conducting visits, you’ll also have opportunities to complete documentation or meetings remotely.
  • On-the-road ready : Expect up to 100% travel throughout assigned markets, bringing care, connection, and compassion directly to participants’ homes.
