Summary / Objective
Primary function is to administer highly skilled private duty nursing care to pediatric patients / clients, in their places of residence; coordinate care with the interdisciplinary team, patient / client, their family and referring agency. This role requires a seasoned nurse, with pediatric home health experience, willing to travel and confident in their skills to initiate care in the patient’s home under the supervision of a registered nurse.

Work Environment :
Works in multiple patients’ / clients’ homes in various conditions; possible exposure to blood, bodily fluids and infectious diseases; must have ability to work a flexible schedule and to travel locally; some exposure to unpleasant weather
Physical / Mental Demands :
Prolonged standing and walking required, with ability to lift up to 50lbs and move patients / clients.
Requires working under some stressful conditions to meet deadlines and patient / client needs, and to make quick decisions and resource acquisition; meet patient / client and family individualized psychosocial needs.
Requires hand-eye coordination and manual dexterity.
Requires ability to adapt quickly to newly established patients onboarding to the agency including new families to homecare or from hospital to home or changing agencies.
Requires ability to work within dynamic team of transitioning care to established private duty nursing team.
Requires ability to teach and train caregivers and other nurses on team.
Requires excellent communication to supervisor, team members, parent, and physicians.

14. Flexibility to provide staffing in variety of patient homes.
