Registered Nurse Care Coordinator (RNCC) - Stroke Coordinator

Clinician with advanced clinical expertise in acute stroke care, recognized by peers as a leader within the institution. Provides expert care and implements treatment plans in collaboration with patients, their families, physicians, and other members of the health care team. Has aggregate responsibility for nursing care, incorporating current literature and research to guide practice. Identifies strengths within the service area and develops opportunities to mentor others, promote professional growth, and improve patient outcomes. Demonstrates expertise in scholarly activities focused on advancing and promoting the profession.

This role serves as the Stroke Coordinator who is involved in clinical and quality care across the stroke continuum. Activities involve assisting acute stroke patients, rounding with the stroke team, and functioning as a stroke clinical expert and resource. The Stroke Coordinator is responsible for collecting, analyzing, and reporting stroke data and metrics; ensuring compliance with evidence-based guidelines and regulatory requirements; providing staff education, and collaborating on performance improvement initiatives to enhance stroke care quality, patient safety, and outcomes.

Individual contributors with responsibility in a clinical discipline or specialty. Typically involves diagnosing, treating, and caring for patients, and often include face-to-face interactions with patients. May also spend some time designing and implementing clinical programs, policies, and services using specialized knowledge and skills. Specialty license or certification required which is acquired through advanced education, training and / or experience. This career stream will be most commonly applied to patient care and / or scientific / laboratory technologist jobs.

Requires basic knowledge of job procedures and tools obtained through work experience and require vocational or technical education. Works under moderate supervision. Problems are typically of a routine nature but may at times require interpretation or deviation from standard procedures. Communicates information that requires some explanation or interpretation.

MINIMUM REQUIREMENTS :

Education : Bachelor of Science in Nursing from an accredited nursing program.

Experience :

Experience : 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Specialty Board Certification strongly preferred

License / Certification : Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

Job requires standing for prolonged periods, frequently traveling, and bending / stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift / push / pull 20 - 50lbs. May be exposed to chemicals, blood / body fluids, and infectious diseases.

Position Compensation Range : $85,820.80 - $113,401.60 Annual
