Job descriptionProvide direct patient care and clinical medication management in an ambulatory care setting Collaborate closely with physicians, nurses, and other providers to optimize therapeutic outcomes Evaluate and monitor drug therapy through clinical assessment and patient interaction Document and communicate findings, recommendations, and treatment plans Contribute to the advancement of pharmacy services and support quality improvement initiatives Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited school Licensed Pharmacist (State of Massachusetts) 2–4+ years of pharmacy practice experience ideal but not required (direct patient care preferred) Current BLS certification Valid Driver's License Board certification (BPS or CDCES) preferred Strong critical thinking, communication, and problem-solving skills Be part of a well-established, growing healthcare organization with a strong community mission Collaborate within a supportive multidisciplinary team environment Work with a diverse patient population while advancing health equity Competitive compensation and comprehensive benefits
