• NH License Required\n-utilize triage templates in Epic to triage incoming patient calls.\n-relay lab and diagnostic results to patients, per PCP.\n-Epic inbasket management, including patient portal gateway messages.\n-Conduct patient assessments via phone, devise care plans, educate patients and \n families regarding treatment plans, collaborate closely with PCP to coordinate \n comprehensive patient care.\n-Will respond to medical emergencies in the practice\n-Support medical assistants with clinical questions and concerns\n-Epic experience preferred or other EHR experience preferred.
