Summary
We are seeking a highly motivated and compassionate Case Manager to join our team. The Case Manager will serve as a vital link between our sober living residents and the clinical services provided at our outpatient center. This role is central to helping clients navigate the practical and social complexities of early recovery, ensuring they have the resources and support to successfully transition to independent living.
Shift Availability
- Full Time : 2nd Shift (3pm-11 : 30pm)
Responsibilities
Actively monitor and document client behaviors and activities to ensure a safe, therapeutic environment, including conducting regular room checks and facility walkthroughs.Consistently ensure client adherence to all program rules and regulations, utilizing monitoring and surveillance to maintain structure and accountability.Provide essential support for program logistics, including assisting with admissions, discharges, and comprehensive client orientations.Accurately administer and document all observed urine drug screens (UDS) and Breathalyzer tests as required by program protocol.Provide immediate and effective crisis intervention and de-escalation when necessary to ensure the safety and stability of all residents.Strictly observe and document clients' self-administration of prescribed medications and immediately report any refusals, discrepancies, or adverse reactions to the clinical team.Conduct accurate narcotic counts and manage the secure storage, receiving, and disposal of all medications in full compliance with regulations.Collaborate with clinical staff to support individualized treatment goals and accompany clients to scheduled appointments, therapy sessions, and program activities.Document all observations and incidents clearly and promptly in client records, maintaining detailed, objective, and timely notes.Actively participate in all required staff meetings and training sessions to ensure competency and adherence to best practices.Competencies
Possesses the ability to conduct comprehensive needs assessments (psychosocial, vocational, medical, legal) and develop individualized service plans for clients.Demonstrates empathy and compassion in all interactions and is competent in using Motivational Interviewing (MI) and other client-centered approaches to enhance internal motivation for recovery.Maintains a strong understanding of confidentiality (HIPAA), professional ethics, and all New Jersey state regulations pertinent to sober living and substance abuse treatment.Exhibits knowledge of co-occurring mental health disorders and the ability to effectively coordinate integrated treatment with the external clinical team.Demonstrates skill in identifying and responding to high-risk behaviors, including imminent risk of relapse, self-harm, or harm to others.Responsible for maintaining detailed, accurate, and timely case notes, progress reports, and discharge summaries in compliance with organizational standards.Has experience acting as a liaison with external systems such as the legal system, probation, or child protective services as required for client progress.Possesses the ability to lead house meetings, educational workshops, or life-skills groups within the sober living environment.Demonstrates thoughtful decision-making and maintains appropriate boundaries while upholding objectivity toward all clients.Combines empathetic guidance with the ability to enforce program rules and facilitate a structured environment that promotes recovery.Qualifications
Bachelor's in Social Work, Psychology, Counseling, or a related human services field.Possession of a Certified Alcohol and Drug Counselor (CADC) credential in New Jersey is not required, but highly preferred.Current CPR / First Aid certification, required.Must pass mandatory pre-employment screenings, including background checks and health screenings.Must have a clean driving record for at least 5 years, and a valid driver's license.Strong passion for and commitment to working with individuals in addiction recovery.Excellent interpersonal, communication, and organizational skills.Ability to engage effectively with clients, demonstrating empathy, boundaries, and accountability.Willingness to learn and manage medication-related duties on the job (training provided).If you're ready to contribute to a mission-driven organization and be a catalyst for positive change, apply today!
