JOB SUMMARY

Belgravia Center partners with Fresno County AB 109 Probation - Realignment Division to provide structured reentry and transitional housing services for justice-involved individuals under active probation supervision. The program promotes accountability and personal responsibility through individualized case management and evidence-based programming. Belgravia also supports Fresno County Pretrial Services and operates the CDCR Long-Term Offender Reentry Recovery Program (LTORR), offering up to 180 days of intensive services focused on life skills, employment readiness, substance abuse education, and healthy relationship building.

The Case Worker III / Group Facilitator plays a key role in delivering individualized and group-based services to program participants. Reporting to the Program Director, this position manages a caseload, conducts intake assessments, and develops case plans addressing criminogenic needs. Responsibilities include facilitating evidence-based groups, coordinating community referrals, monitoring progress, and maintaining compliance with contractual and regulatory standards. This role requires strong case management and facilitation skills, sound judgment, and the ability to build constructive relationships while ensuring accountability.

ESSENTIAL JOB RESPONSIBILITIES

  • Maintain a caseload of up to 20 residents, providing individualized case management and support aligned with program goals.
  • Support residents in achieving their case management objectives through comprehensive services, including individual sessions, structured activities, and group facilitation.
  • Conduct resident orientations and complete thorough needs assessments to inform individualized service planning.
  • Educate residents on program procedures and expectations, reinforcing accountability and promoting successful reintegration.
  • Develop, review, and update resident case management plans regularly to ensure progress and goal alignment.
  • Facilitate a minimum of two evidence-based group sessions per week, promoting personal growth, accountability, and successful reentry outcomes.
  • Plan, coordinate, and implement structured group activities that reinforce program objectives and resident engagement.
  • Support residents with exit planning, ensuring connection to ongoing housing and community resources for continued stability.
  • Monitor and document resident progress toward established goals, maintaining consistent communication with referral sources regarding achievements, barriers, and overall status.
  • Maintain accurate, timely, and compliant documentation, including case notes and case management plans, in accordance with agency Scope of Work and CDCR / Probation requirements.
  • Coordinate referrals to community resources and ensure linkage to supportive services that address criminogenic needs.
  • Handle, store, and transmit Protected Health Information (PHI) and Personally Identifiable Information (PII) in compliance with HIPAA and agency policies.
  • Remain flexible and adaptable to changes in workload and assignments.
  • Will think and act quickly and efficiently in emergencies.
  • Will drive on Agency business.
  • Physical presence at the program is required.

ESSENTIAL JOB REQUIREMENTS

  • Possess either a bachelor's degree in social sciences or a related field; or a high school diploma or equivalent and a minimum of two (2) years' experience working with a criminal justice or related population
  • Ability to understand and utilize the Automated Reentry Management System (ARMS), preferred.
  • Current certified training in emergency First Aid and CPR procedures, preferred.
  • Ability to write substantive and grammatically correct reports.
  • Strong interpersonal and communication skills to work effectively with residents, employers, workforce partners, and multidisciplinary staff teams.
  • Ability to collaborate effectively with multidisciplinary teams, including case workers, monitor staff, program leadership, and external partners.
  • Understand and follow complex oral and written instructions.
  • Maintain accurate records, files, and databases.
  • Ability to maintain professional boundaries and exercise sound judgement in high-pressure or emotionally charged situations.
  • Operate modern office equipment, including computer equipment and specialized software application programs.
  • Prepare, review, and present reports, recommendations, and other correspondence and communications in a clear and concise manner.
  • Be on-site at the assigned location during regularly scheduled work hours.
  • Must complete minimum required hours of assigned training on an annual basis.
  • Ability to communicate effectively in written and spoken English.
  • Adhere to HIPAA regulations and agency protocols for handling Protected Health Information (PHI) and Personally Identifiable Information (PII), ensuring confidentiality and data security.
  • Ability to work effectively and professionally with staff and program participants.
  • Experience in de-escalation techniques, trauma-informed care, or behavioral intervention strategies, preferred.
  • Intermediate computer skills in Office 365, Teams, Zoom and other communication / meeting platforms.
  • Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a pre-employment criminal background check.
  • Ability to pass a pre-employment drug screen, and TB test.
