Behavioral Health Care Planner

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

Provide comprehensive support to behavioral health clinical teams and facilitate member engagement

Educate members about specialty programs and assist them in locating services

Collect information on patient progress and perform miscellaneous administrative duties

Required Qualifications

2 years of Behavioral Health care experience

Previous Customer Service experience

Proficiency with computer skills, including navigating multiple systems

Associates Degree or equivalent combination of education and experience
