They provide direct care to treat the medical / surgical condition through the use of accepted physical therapy modalities.The Physical Therapist also Instructs and supervises the patient / family and home health aide (and others) in the home exercise program that they write and other aspects of patient care as appropriate and documents their response to instruction.Continually re-evaluates the patient’s condition and modifies the plan of treatment as appropriate.Our PT identifies and reports significant clinical information to, physician, clinical manager, primary nurse coordinating services and other disciplines.
