Promise Community Liaison - Senior Services

POSITION SUMMARY

The Promise Senior Services Community Liaison is a frontline staff who works with individuals from diverse backgrounds and has a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison between social services and the community to facilitate access to resources and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing knowledge and self-sufficiency through a range of culturally and linguistically appropriate activities and services such as outreach, community education, and advocacy. This position is designed to work with SSL residents 55+ years of age.

ESSENTIAL RESPONSIBILITIES AND DUTIES

1. Community Outreach & Engagement

• Assess needs and facilitate connections to services, programs, and resources for seniors, including those who may be unserved or underserved.
• Work with seniors, senior housing and care facilities to creatively solve obstacles to accessing resources such as health care, food security, education, and employment.
• Provide mental health resources and information at community events and activities.
• Conduct home, office, and public location visits to assist seniors and their families, ensuring equitable access to outreach programming.
• Monitor changes in senior clients' situations and provide follow-up assistance as necessary.
• Assist clients in applying for financial support and refer them to appropriate sources for additional services.
• Promote, and attend community engagement events, such as PSSL quarterly family nights and citywide events.
• Recruit senior residents to participate in a variety of community councils, coalitions, and focus groups.
• Identify ways to connect or build a social infrastructure to support SSL seniors.

4. Participate in the South Salt Lake Cares Coalition (Education, Health, Arts, Neighborhood, Jobs and Economy, Safety, Housing, Interfaith, and Social Impact) and

supervise City events as assigned.
